  • Completed forms, declarations, and any required supporting explanations
  • Evidence that connects the request to the applicant's current status, purpose, and immigration history

Do not send passport numbers, UCI, SIN, or sensitive identity documents through a public form. Bring sensitive records to a private consultation workflow only when requested.
